Proof of Address: Any mail or bill with your name and current address

Proof of Income: Pay stubs within the last 30 days or last year's W2 or Medi-Cal Card

Proof of ID: Driver's License, Social Security Card, or Medical Records (Birth Certificate)

Foster Families: If you are a legal guardian, please submit proof of care/custody.

Military Families: Please submit your W2 or LES as proof of income.
