If you are pregnant, submit your proof of pregnancy.
If you have an active Medi-Cal, submit your Medi-Cal card as proof of income.
Proof of Address: Any mail or bill with your name and current address
Proof of Income: Pay stubs within the last 30 days or last year's W2 or Medi-Cal Card
Proof of ID: Driver's License, Social Security Card, or Medical Records (Birth Certificate)
Proof of Pregnancy: Positive test confirmation from your health care provider or ultrasound
Foster Families: If you are a legal guardian, submit proof of care/custody.
Military Families: Submit your W2 or LES as proof of income.
