Our Goal

Our goal is to enhance healthcare access for these communities by offering care coordination, telehealth services, and helping manage chronic conditions like diabetes and hypertension.
Key Objectives

Establish a collaborative Network of Care with hospitals, healthcare providers, and social service agencies to facilitate care coordination.

Equip Avenal and Corcoran Public Health Clinics with ADA-compliant telehealth rooms to facilitate remote consultations.

Community Health Assistants (CHAs) will deliver direct, in-person services to assist with care coordination and connect individuals to necessary resources.
