What is LICN?

  • The Local Indigent Care Needs (LICN) Program is a grant-funded initiative aimed to improve healthcare access for underserved adults in Kings County, particularly in Avenal and Corcoran.

Target Population
  • Individuals facing housing and transportation barriers

  • Individuals managing chronic conditions such as diabetes & hypertension

Our Goal
 

  • Our goal is to enhance healthcare access for these communities by offering care coordination, telehealth services, and helping manage chronic conditions like diabetes and hypertension.

Key Objectives

  • Establish a collaborative Network of Care with hospitals, healthcare providers, and social service agencies to facilitate care coordination.

  • Equip Avenal and Corcoran Public Health Clinics with ADA-compliant telehealth rooms to facilitate remote consultations.

  • Community Health Assistants (CHAs) will deliver direct, in-person services to assist with care coordination and connect individuals to necessary resources.

The Impact
  • Increased access to healthcare will address barriers like transportation and homelessness, improving chronic disease management and health outcomes.

  • Strengthened partnerships between healthcare providers and community organizations will ensure comprehensive support, reducing disparities in underserved populations, while also improving efficiency by streamlining service delivery and reducing redundancies
